Forms and Documents for Congregational Reporting of Financial and Insurance Plans.

Certification of Audit and Blanket Insurance Forms
Each year in late January each congregation’s treasurer is asked to complete important documents to ensure we have accurate documentation on financial and insurance plans. Two of these forms are the Certification of Audit of Financial Records on Congregations and the Blanket Employee Dishonesty Coverage Liability Insurance Renewal.

Remittance Form
Annually, we provide remittance forms to each congregational treasurer to complete when submitting Mission Support to the ELCA South-Central Synod. Each congregation receives a form with the name of their congregation and their congregational ID number included. It is important to include the form with your remittance. Also, provide the date submitted; check number, amount, and explicit detail of where the monies are to be designated. Please be specific when designating Mission Support, and/or other support, such as, Hunger, Lutheran World Relief, Disaster Response, Seminarian assistance, Missionary Sponsorship, and Missional funding.
